"Antler Carved Flute"

Description:
UA-005 is a woodwind flute instrument carved from the antler of an unknown creature. As of 03/20/1986 the flute structure has started to deform due to crackage. When an individual plays the instrument regardless of skill, a large antlered rabbit appears from a cloud of mist summoned by the flute. The rabbit has been designated as UA-005-1. UA-005-1 is docile in nature and acts similar to a standard rabbit despite the antlers extruding from the head, these antlers have the same cracks as UA-005 itself and are linked together. It is believed that UA-005-1 is memeticly capable, as when was summoned facility personnel could report hearing a voice speak to them in their minds.
The summoning UA-005-1 causes all nearby individuals to hear a voice saying the rhyme "Starlight Starbright" and asking for their "deepest wishes". Only the person who summoned UA-005-1 is able to have their wish obtained. Information obtained from UA-004 and the evidence from the forming cracks in UA-005 suggest that there is a limit of 5 wishes to be granted. After the summoner requests their wish UA-005-1 anters will begin to glow following with the granting of the wish(Not yet entirely proven). After granting the wish cracks will form on UA-005-1 and the entity will soon vanish from the area.
UA-005 Wishes
Head Researcher Permission is required to grant a wish from UA-005. Unauthorized use of UA-005 will result in dire consequences.
First Wish:
D-4470 was the first to utilize and have a wish granted during initial testing of the object. What D-4470 had wished for is unknown as they were unable to be heard by testing personnel. After UA-005-1 Antlers began to glow a mist surrounded D-4470 which had caused them to vanish. A complete security sweep of the facility had been conducted and D-4470 was no where to be found. It is currently unknown if D-4470 alive or not, but it can be presumed that they had wished for some for freedom.
Second Wish:
Senior Officer Hernandez was granted the second wish. For unknown reasons during the security sweep searching for D-4470 Officer Hernandez had entered the chamber and played UA-005. Officer Hernandez was overheard saying "Starlight, Starbright, Please grant me a wish here in tonight. I wish... I wish for civilian life, to forget all this... to see my family again". Amist had surrounded Officer Hernandez causing them to vanish. Currently it is unknown if Officer Hernandez is alive, or their location.
It is recommended that a field agent attempt to investigate based from the information in Officer Hernandez's profile to see where they may have ended up.
Discovery:
UA-005 is a "gift" recieved from UA-004. The flute had been brought into the facility by UA-004 during the first apperance. Conversation with UA-004 revealed that they are the finder of UA-005 and not the creator.
