Description:
UA-004 is a humanoid individual that is covered in an unknown black substance standing approximately 6'-6" tall. UA-004 wears worn and tattered clothing, an old cloak, and a smooth mask made from an unknown material. The entity summons itself from an old wooden mask currently stored in lower R-wing. The voice of UA-004 is incredibly strong capabable of causing infrastrucural damage it is recommended to only keep vocal commuincation with UA-004 to a minum. The entity is not entirely hostile and is capable of speech although caution is strongly adviced. The unknown substance that covers UA-004 is extremely hazardous and capable of causing severe injuries to persons that make physical contact with the substance. UA-004 is capable of phasing through enviornments and utilization "teleportation" through the use of the substance. UA-004 was also observed to manipulate gravity being able to pick up humanoid objects and crush them to death, it is unknown how UA-004 is capabable of such abilities.
On 03/21/1986 after concluding testing on UA-005, UA-004 had made an appearances commenting on the use of the flute. When questioned about attempting to utilize UA-005 to fullfill it's request for life UA-005 responded saying "I tried, It took everything. I made my wishes... they brought me bliss... only to find out... it was death that I kissed...".
Observation of the substance under a mircoscope had shown that the substance is made from several teeth and purple eye like strucutres, continued observation of the substance is believed to cause a memetic effect on the observing persons. The substance is sentient and violent, during observation the black goop had breached containment on it's own leading to the immediate dissinfection of the R-wing laboratory. Facility staff had observed the substance attempting to crawl into the orfaces of testing research personnel, the substance had to be medically removed by staff.
Discovery:
On 03-14-1986 UA-004 made a sudden appearance inside of the facility causing a lockdown to be initialized by area security. When UA-004 was spoken to by area staff it was revealed that a deal was made with the UA by an unknown person from another foundation facility. It is likely that this facility was the Area-48 Siberian location although not confirmed. UA-004 claimed that gave gifts and in return wanted to be brought to life. The descirptions of the three gifts each gift matching a previously discovered unlogged UA entities, "The Ring, The Flute(UA-005), The Seed". All of these entities are located and stored in lower R-wing although giving them to UA-004 is not advised due to their unknown agenda. During the lockdown UA-004 appeared in D-block before vanishing and leaving a crate that contained a wooden mask, and a white flute, these items have been placed in R-wing storage. Currently it is believed that UA-004 wants to be fixed/brought to life in return for the exchange of its gifts. "The lens and the moon" are unknown items that UA-004 claimed to be a part of them, it is unknown what these parts are from description alone more research is required into the matter.
