Incident Report #010 "UA-003 Breach" - 04/05/1985

During a routine test of UA-003, security was requested to transfer UA-003 to another testing chamber for continued testing. While security was preparing to secure the anomaly, the door was opened by Janitor Dang opened the door prematurely, causing the anomaly to breach containment. Security tranquilized the anomaly and attempted to place it into a containment cube, however, the anomaly evaded security and eventually breached the surface. In a vain attempt, security found the anomaly before it burrowed into the earth, unsuccessfully as it buried itself into the ground.

UA-003 has breached containment and is on the surface.


Addendum: Janitor Dang Interrogation - Level 4 Clearance Required to View

Following the incident, Janitor Dang was detained by Medical Director Kingsley and transported to the interrogation room. Doctor Bee was the first responder and acted belligerent towards the Janitor due to him ruining her test. Later, Engineering Lieutenant Odetted was called to the interrogation due to her being the superior of Janitor Dang. Both Manager Haas and Head Researcher Smooth also attended the interrogation. It was believed that Janitor Dang was responsible for prematurely opening the door and causing the anomaly breach, the camera footage confirming that idea, and a statement from Odette. Instead of admitting to the involved personnel about opening the door, he lied about it. Manager Haas and Head Researcher Smooth chose to demote the janitor to D-Class. At this point, Janitor Dang fled from the interrogation room, the door open, he closed it on the personnel in the interrogation room. The door was opened after a few moments and Medical Director Kingsley, Manager Haas, and Doctor Bee took chase. Janitor Dang ran into HCZ, and at which point, Medical Director Kingsley used lethal force, shooting the janitor with a UZI SMG 11 times.

During the interrogation, Janitor Dang was spoken to brashly, and was never able to get a word in to defend himself. No motive was established, and punishment was handed out to him quickly. Further review of the camera footage by the Chief of Security Danger showed that Janitor Dang was struck in the head with a crowbar by an unknown engineer, likely causing some form of concussion, leading to a heavily altered mental state, likely the cause of his actions.

Deadly force was used on the subject after he ran into the HCZ, not stopping for the pursuing personnel. Medical Director Kingsley, at the time, believed that the subject was going to attempt to breach an SCP from the HCZ due to his demotion to D-Class.

This incident is a good example of why it's important that interrogations are conducted properly. None of the proper procedures were done and those interrogating Dang did not have the authority to do so. Because of this there are a lot of unanswered questions, a missing UA and a dead member of our personnel. I want it to be clear for any Level 4's reading this that Security is the only branch that conducts interrogations, specifically Provost. Do not involve yourself unless requested, because as we saw it ended poorly. Final decisions regarding punishment of a subject is also made by Security. I'll be making sure this is enforced if future events occur. On a second note please leave all emotions out of interrogations as well, it does not benefit the process in any way.

Do not speak of performing interrogations properly security chief. You would have put a bullet in the back of their head for a lesser matter as we already know from previous incidents. Mind your hypocrisy.

The best to do from this situation is to treat the cause, not the symptom. In doing so unprofessional, rowdy, and useless behaviour will not be tolerated in testing situations. This extends to all staff security included.

Reading this gives me a headache. In addition to the other research log, everyone involved here is capable of handling a incident like this without the need of myself or my MPs. When it comes to interrogations yes, my detectives are trained to do so. However to this extent SOC are able to conduct it as they see fit as well if none of my staff are available. The termination of the Janitor while bearing no fruits for us to investigate was done rightly. The risk of a SCP object being released in the Heavy Containment Zone would have resulted in more casualties and put the Area on lockdown. In the future I urge you to at the minimum get a member of SOC and a host of security personnel to guard the area incase of certain "events".

As for the test and the nature surrounding the escape of UA-003 I feel as if it was unavoidable. No one can account for ones mental health on the spot nor spot any abnormal behaviors should a suspect show it. It's what makes my Divisions job so difficult. So focus on this.. We are the only line of defense against bad actors comrades. It is up to every single one of us to do the jobs assigned to use or othewise... We all fail.

There will be no follow up investigation into the matter other than identifying the rogue agents still within our walls. Be vigilant comrades, and trust no one.

To clarify my message I meant that Security in general handles investigations, Provost if they are present. That means my SOC and not researchers. I told Kingsley this in person and they will be prepped incase something similar happens again.
